- INCONSISTENT NAMING: the Oklahoma State Department of Health licenses this facility as the 'OTTAWA DETENTION FACILITY' — dropping the word 'County' — and it carries provider ID DET-001, the first in the state's series. The county and sheriff give the jail no proper name at all.
- The county runs the same website under two domains, ottawaok.gov and ottawa.okcounties.org, which cross-link inconsistently; some navigation links point at one domain and some at the other.
- The 12/29/2025 state inspection found the facility not in substantial compliance and records that the Jail Administrator did not produce records when they were requested.
- The Assessor is listed as Rebecca Brewer on the county site but as Becky Smith in the Oklahoma State Election Board's 2024 roster — an example of names on this county's pages drifting from the state record. The Sheriff's name is consistent across both.
